Schedule Changes Dec. 4 & 5

Parents & Guardians:

Please fill out the form at the link below to request changes for your student's schedule. We will do our best to accommodate as many as requests as possible. Sometimes changes are not possible due to classes being full, conflicts with other required classes, among other reasons. If we are able to make the change to the schedule and it was not due to an error, a $10 fee will be added to their myDSD account and you will see that change reflected by 4:00 pm on Tuesday, December 9th.

PLEASE READ BEFORE COMPLETING FORM: We will NOT be making any teacher changes. If you are wanting to request to change more than 2 classes, please put those requests in the Additional Information section.
